TRANSITION GUIDE SOUTHERN CALIFORNIA FIRST

From treatment to housing: plan the practical details.

The time between a program and the next residence can involve several independent decisions. A written plan can make those steps easier to understand.

Clarify the kind of setting needed

Separate housing from clinical services

Detoxification, residential treatment and recovery residences are different categories. A sober-living home generally does not substitute for medical supervision or licensed treatment.

Discuss individual clinical needs with the treating team. Use general public directories to identify questions, not to diagnose what level of care is appropriate.

Create a housing questions sheet

  • General location and transportation needs.
  • Expected cost and how it will be paid.
  • House rules, roommate arrangements and basic amenities.
  • Operator contact and a direct way to verify open space.
  • Transition timing, move-in logistics and required nonclinical documentation.

Confirm responsibilities

Who verifies each fact?

Ask the treatment team what information it can provide, ask the housing operator to confirm policies and openings, and avoid assuming a directory listing represents a reservation or acceptance.

This guide does not process resident information. Welcome Home Bridge does not currently coordinate handoffs or referrals.

Keep an up-to-date reference

Recheck before making commitments

Availability, costs and policies can change quickly. Record the date you confirmed practical details with the provider and revisit them before making arrangements.
