Housing Research Checklist for Discharge Teams

Discharge planning can involve practical housing research. This worksheet distinguishes public operator facts from patient-specific assessments, clinical decisions, and protected records that must remain outside this website.

Define the research scope before contacting an operator

Start with a city or geographic region, general type of residence, expected transportation resources and the business questions you need answered. Do not create a named patient search, enter an individual’s treatment details or transmit a discharge packet through Welcome Home Bridge.

  • Record the scope in non-identifying terms such as county, operator category and public service area.
  • Keep any person-specific care planning exclusively in existing authorized systems and professional workflows.

Verify the operator and the right business contact

Identify the operator's publicly advertised name, business website, phone, exact published property or service area, and the person authorized to clarify general house policies. Record when the details were checked and whether they came from a government source, operator statement or independent certification record.

Distinguish housing from regulated treatment

A sober living residence and a licensed residential substance-use treatment facility are not interchangeable. Check California DHCS licensing only for services to which DHCS licensing applies, and confirm any voluntary recovery-residence certification with its issuing body. A search result is not proof of clinical suitability or available housing.

Prepare a consistent nonclinical operator question set

Ask whether the operator can supply a current written housing cost schedule, move-in charges, refund terms, house rules, staff or peer-support description, general accessibility information, transportation policies, geographic location and business correction channel. Mark unknown answers as unknown.

  • Do not represent a business answer as a patient-specific approval.
  • Do not infer live vacancies or reservation rights from a dated directory entry.

Check location and logistics without personal data

Compare the actual city, transit routes, approximate commute demands and ordinary access to community services. County-wide listings can conceal large travel distances. Verify real addresses or route feasibility using authorized sources when an operator permits that information to be used.

Document attribution, updates and open questions

Maintain an internal business-only research table with operator name, source link, date checked, confirmed general policies and pending verification questions. If public fields are wrong, use the site’s directory correction route and refer source-owned errors to the publisher. Do not move referral records into this document or the public inquiry form.

Keep the clinical decision boundary intact

Determinations about an individual’s placement, treatment needs, eligibility, consent and discharge remain with the qualified, authorized care team. Welcome Home Bridge currently provides only public research and organization-level pilot interest, not a patient-handling workflow or referral exchange.
