THE MODEL SOUTHERN CALIFORNIA FIRST

Better information.
More intentional transitions.

Our approach has two parts: public, source-attributed discovery now, and carefully controlled provider collaboration as a future product stage.

Platform in development · No treatment, referral, placement, or real-time availability service is currently offered.

Start with information you can verify

01 /

Explore a directory

Browse the public treatment registry view and recovery-housing discovery resources, organized for Southern California.

Explore directories ↗
02 /

Compare relevant questions

Review the types of policies, housing features, cost details, and program differences worth asking about before making decisions.

Use our checklist ↗
03 /

Confirm with the source

Use official directories and contact the organization directly to check current licensing, services, fees, eligibility, and availability.

04 /

Make informed plans

Keep clinical decisions with qualified professionals and housing acceptance decisions with providers. No automatic placements are made.

What is planned—not yet operating

A future nonclinical workflow

The Welcome Home SaaS roadmap includes structured provider profiles, possible availability reporting, permission-based communication, and handoff coordination between participating organizations. Those functions are not open to the public and remain subject to legal, security, vendor and operational release gates.

We do not yet support patient referrals, waitlists, resident onboarding, clinical interoperability or booking through this website.

Important distinction: This is not a treatment matching service, a referral service or a substitute for licensed care. For help finding treatment, use SAMHSA FindTreatment.gov ↗.
